Project Title: Strengthening health system to establish a care of chain and referal for management of non-communicable diseases in hard to reach population in Tanzania

Project Description: Non-communicable diseases (NCDs) such as arterial hypertension, heart failure, diabetes, chronic respiratory diseases, and cancer are responsible for a substantial burden of disease in sub-Saharan Africa. The awareness of NCDs in the population is poor. Patients with NCDs are often diagnosed in an advanced stage of the disease, and mostly are not treated. The result is a high mortality due to NCDs. Actions to fight NCDs in sub-Saharan Africa have recently been initiated on international and national levels. The overall objective of CARECHAINNCD is to improve NCD care in rural and hard-to-reach populations in the Kilombero region in central Tanzania, and in the Bagamoyo District in the coastal region of Tanzania. This objective is achieved by establishing an integrated protocol-based care and referral system for NCÜs for the rural population attending medical care at healthcare center and village dispensary level. This NCD care system is complemented by NCD training- and prevention components. As a result of the proposed project, more patients will be aware of their disease, more will be diagnosed, more will be referred to specialized centers, and more will be treated. A central aim of the program is the ensure the availability of affordable drugs for the rural population by cooperation with the government and the Novartis access program (aim: t. This project will be carried out in collaboration with the Ministry of Health, Muhimbili University of Health and Allied Sciences, Ifakara Health Institute, Saint Francis Referral Hospital, Good Samaritan Cancer Hospital, Ifakara, Bagamoyo District Hospital, Bagamoyo, Tanzania, Novartis Global Health, Swiss Tropical and Public Health Institute, Basel University Hospital, Switzerland, and Charite University Hospital, Berlin, Germany.
